There is a lot going on there with the elbow and forearm when you punch. Yes the repeated impact and yes the motion of the elbow joint itself every time can cause inflammation. Also making a fist flexes all the muscles in the upper forearm where they connect to the elbow joint. This can add to the problem. These all can cause tendonitis and general elbow problems. Golf and tennis are known for causing elbow problems as well. You may need to back off it for awhile and rehab till better. figure out what exercise is the worse and stop for awhile, see if it improves. Double end bag and shadow boxing are both lower impact alternative to the heavy bag.

Hold your arm straight out in front of you then use your other hand to pull the fingers on your arm back until you feel your forearm tighten. Do this about 10-20 times.

Are you doing resistance exercises like chins, pullups, curls? I've gotten it a few times from those movements.

Yeah I'd say its probably from improper mechanics on the jab. I had a similar issue. You shouldn't be snapping it out at the elbow to try to snap it out there quickly its more in the shoulder. Make sure you are extending out straight forward as well to begin the punch.
I used K tape or a compression sleeve for my elbow while I had it. I worked on the jab mechanics a bunch with my coach and I don't have the issue anymore.
